Resources for Lung Transplant Patients in myUHN Patient Portal

Your care team may enroll you in a care plan to help you manage your lung transplant. You can follow this care plan through your myUHN patient portal account. This care plan is an interactive, personalized plan of care delivered through myUHN and the MyChart mobile app. You will receive real-time notifications for things such as:

Messages from your care team
Health-tracking tasks
Educational content
