Patient Relations

​​Would you like to share feedback about the care you or your loved one received at UHN? Whether you have questions, a complaint, a suggestion or a compliment, we'd like to hear from you. Your feedback is important – we want to learn what UHN is doing right and where there are opportunities to improve.

We encourage you to speak with your health care team (or a Manager) about any questions or concerns before contacting Patient Relations. Your health care team may be able to act on your concerns and answer any health care-related questions right away. If more support is needed, visit our Contact Us page.

How can Patient Relations help me?

If you want to share feedback about your care, one of our Patient Relations Advisors can:

  • Provide a caring environment for you to share your experience
  • Listen to you and make sure your comments, questions or concerns are heard
  • Document information about your experience and share your feedback with the right people in the organization
  • Help find answers to questions you may have about your care
  • Support communication between you and your health care team

Please note: if you are a family member, caregiver and/or friend submitting feedback, we may ask to speak with the patient. If the patient is not able to speak with us, we will speak with their Substitute Decision Maker (SDM).

Our goal is to make sure the care you receive is high quality, compassionate, fair, and aligned with UHN's Patient Declaration of Values.
