How to Refer a Patient

The Centre for Mental Health (CMH) has two primary pathways for referring a patient – one for outpatient; one for inpatient.

  1. Outpatient Referrals (internal and external)
  2. Inpatient Services (internal UHN only)

Eligibility criteria vary by clinic. Please consult specific services by clicking on the links below for guidance. If you are uncertain about the appropriate clinic for your patient, please fill out the referral form and the triage team will review.


Outpatient Referrals: Refer a Patient through the CMH Central Referral Intake Service

To complete the Central Referral Intake process:

  1. Download and complete the Central Referral Intake Form
  2. Submit the completed form via fax to 416 603 5215. The form will be reviewed to ensure that it is complete; if incomplete, the form will be returned to you.
  3. Once the referral is accepted, the patient will be contacted by a clinic to book their first appointment. Information about the appointment will also be sent to your office as per the CPSO guidelines.
