What We Do

Geriatric Psychiatric Clinics work in partnership with patients' primary care and other specialist providers to offer the specialist support of geriatric psychiatrists. Geriatric Psychiatry Clinics provide comprehensive geriatric mental health assessments, and care planning recommendations and services; links to other specialist, home and community mental health services and programs like Reconnect Health Services.

Patients accepted for service through UHN Geriatric Psychiatry are also offered Reconnect's community-based services where they are eligible. These services vary in intensity based on needs. Case managers may connect patients with a crisis hotline for seniors, with staff who will meet with clients in the community; family and caregiver support programs; or a variety of programs that work towards connecting clients to the services that will support their mental health. These services can include getting a doctor, getting an assessment, avoiding further contact with the justice system, managing addictions, or getting information about housing.
