To transfer your prescription records to the Transplant Outpatient Pharmacy, please call 416 340 4415 and select option 4 to leave a voice message with the following information:
- Patient first and last name
- Patient date of birth
- Phone Number
- Current Pharmacy Name
- Current Pharmacy Phone Number
- Prescription number(s) and/or
- Medication name(s)
One of our pharmacy staff members will contact you once the transfer of your prescription record(s) is complete. Please allow up to 2 business days for transfer.
