How to Get Referred

You need a referral from a physician to become a patient at the Thrombosis and Hemostasis Clinic.

When making a referral, the physician will include:
  • Completed referral form
  • Clinical notes, including history and reason for referral
  • List of current medications
  • Recent blood work
  • Ultrasound/CT/MRI test results
How Long Does a Referral Take?

The referral needs to be accepted and triaged by the medical staff. Once the referral is triaged, a member of the clinic's administrative team will call you directly with an appointment. Please ensure that your contact information is up-to-date as we will also send you an itinerary for your appointment. This itinerary will include information on where to go for your appointment.
