Attention: Toronto Rehab Outpatients

We are changing our outpatient services during the COVID-19 outbreak. This is for the safety of you, other patients and our health care teams.

Your clinic will be in touch with you, to either reschedule your appointment for a later date, or to discuss options for continuing your therapy via video conference or telephone consultation.

If your condition requires urgent attention, please call your clinic to discuss your care options.

When we return to our regular services we will re-evaluate your therapy needs, to determine if any ongoing therapy will be provided.

Thank you for your support as we continue to work together for everyone’s safety.

– Your Outpatient Care Team

Please Note - Due to COVID-19 precautions to ensure the safety of our residents and staff, Lakeside Centre is currently closed to visitors.
