Welcome to the Cardiovascular Prevention and Rehabilitation Program where we help patients with heart disease improve their cardiac health and fitness. This program goes beyond recovery by helping patients reduce their chance of future heart events through lasting lifestyle and behaviour changes.

The program is designed for patients with a history of heart attack, heart surgery, valve surgery, angioplasty, arrhythmia, angina, stroke, heart failure, or other heart conditions. It's also for those at high risk of heart disease because of their family history, high cholesterol, high blood pressure, or diabetes.

The program is hosted in part at Toronto Western Hospital and has specialized programming at Toronto Rehab — Rumsey Centre that uses the Cardiac College – Living and Thriving with Cardiovascular Disease as a framework for discussion and behaviour change.
