What We Do

Toronto Rehab’s outpatient rehab service offers, assessment, consultation, treatment and education to people who have experienced a stroke.

Therapy may be offered on a 1:1 basis, in groups, and through telehealth, when needed.

Therapy is tailored to meet each patient’s own needs and goals.

Our interprofessional team works with patients and their families to help meet their rehab needs and goals toward successful community re-engagement in the following areas*:

  • Caregiver support (caregiver coping and access to resources)
  • Communication (the ability to express needs and participate in social interactions)
  • Environment (home modification, alternate housing)
  • Financial management (the ability to manage financial affairs, access to funding sources)
  • Health management (nutrition, prescriptions, mood)
  • Life roles (return to work, school, volunteering)
  • Mobility (aids and equipment, return to driving)
  • Social networking (relationships with family, friends, contacts with agencies)

Patient Education Groups are also provided several times a year across the Rumsey Centre site and the University Centre site.

(*These areas are adapted from the Stroke Trigger Tool, Heart and Stroke Best Practice Guidelines for Stroke Care.)
