Urgent Referral
To complete the referral process:

Include the following documents:
- History of present illness
- Previous medical history
- Medical list including all allergies
- All recent medical imaging
- All recent lab work

Fax the completed form to 416 946 2043. Referrals should be submitted directly to the Wallace McCain Centre for Pancreatic Cancer without specification of physician via fax. If you would like information on the McCain physicians and surgeons, please contact the Centre directly:

Our office will send you a confirmation of receipt. The referral package will be reviewed to ensure that it is complete; if incomplete, we will contact your offices for further information.

You will be contacted with information about the patient’s first appointment within 2 weeks. We will also contact the patient directly with the appointment information, including time location, and important documents, test results and imaging that they must bring.
Patients are asked to bring a family member with them to their appointments.
It is extremely important that your patient brings the following to their first appointment:
- Past medical history
- History of present illness
- Medications in original pill bottles
- List of questions
Please note: The Wallace McCain Centre will contact patients with their appointment time within 2 weeks if all appropriate information has been submitted by their doctor. Patients are usually seen within 2 weeks once a complete referral is received. Requests for a consultation with a Medical Oncologist may take up to 3 weeks, if the clinic has received many referrals. If the patient does not receive information about their appointment within 2 weeks, the referring doctor should follow up with the Wallace McCain Centre.
