​The Patient Declaration of Values is a statement of our commitment to our patients to live out and practice these values in the care we provide.

This document is more than just a checklist of values. It is the result of a partnership between our patients and staff, who all contributed their ideas to the final product. In fact, this Declaration is largely in the words of our patients—over 4,000 of whom were surveyed for their input.

The results were remarkably consistent—what our patients asked for, and how they phrased it—are the words that are used in the final document.

UHN has, in fact, been living these values for a long time already. If you have visited UHN before, you might recall that we have had such statements in the past. For example, years ago we called it the “Patient Bill of Rights and Responsibilities” and then this document was modernized and called the “Working Together Document.” UHN has always been committed to recognizing the importance of patient-centred care and the voice of our patients.

Patient Bill of Rights and Responsibilities

At UHN, we believe that patients are our partners in their care. The safest and highest quality care is achieved when patients/families and staff work together collaboratively as partners.  Thus, the Patient Bill of Rights and Responsibilities was developed, with input from both staff and patients of UHN, to set clear expectations for all.
