What We Do

The GIM Program at TWH focuses on best practice in acute medical and senior friendly care. The inter-professional team meets daily to discuss and update each patient's plan to ensure a seamless journey.

The majority of patients have diagnoses of:

  • Diabetes
  • Congestive heart failure
  • Pneumonia
  • Gastrointestinal bleed
  • Acute kidney injury
  • Chronic obstructive pulmonary disease
  • Rheumatologic disorders
  • Heart disease

You need to be seen and admitted by a doctor at UHN to become a patient in one of our inpatient units.
