Antimicrobial Stewardship Program

Carly obtained a Master of Science degree in Clinical Microbiology from Griffith University in Brisbane Australia. She started her career in research in a microbiology lab at Sunnybrook Health Sciences Centre. She then moved into the field of infection prevention and control in 2007 at St. Joseph's Health Centre in Toronto. Carly joined the UHN Infection Prevention and Control (IPAC) Department in 2009 as an Infection Control Practitioner (ICP). She then worked as an ICP in New York City gaining insight into the American healthcare system, returning to Canada and the UHN IPAC department in 2015 and becoming their manager in 2018. Carly joined the ASP team as manager in 2022 and later became the Director of IPAC and ASP in 2023.

Dr. Shahid Husain is a renowned expert in Transplant Infectious Diseases and professor of Medicine at the University of Toronto. He completed his Internal Medicine residency at Cook County Hospital, followed by a fellowship at the University of Pittsburgh Medical Center (UPMC). After serving as faculty at UPMC, he joined the University of Toronto in 2008.

Dr. Husain has led international transplant infectious disease councils and has contributed to medical guidelines development. His research focuses on infection risks and antibiotic use in transplant recipients, supported by major health organizations, with over 200 peer-reviewed publications. He established the largest Transplant Infectious Diseases Fellowship program and currently directs the Antimicrobial Stewardship Program at the University Health Network, mentoring future specialists.

Dr. So received her Doctor of Pharmacy (PharmD) degree from the University of Toronto and Master of Public Health (MPH) in Epidemiology from the Harvard T.H. Chan School of Public Health, Harvard University. Miranda is a Clinician-Investigator with the Toronto General Hospital Research Institute, Assistant Professor (Status) and Graduate Faculty Associate Member at the Leslie Dan Faculty of Pharmacy, University of Toronto, and Lecturer (Clinical-Honorary), Division of Infectious Diseases, Faculty of Medicine, Dentistry and Health Professions, University of Melbourne, Australia. Her work focuses on antimicrobial stewardship in immunocompromised patients, public health and the contributions of pharmacists. Miranda chairs the Canadian Society of Healthcare-System Pharmacy Foundation Education Grant Committee and serves on two editorial boards, Transplant Infectious Diseases and Clinical Microbiology Reviews.

Dr. Howard Song received his combined MD and PhD degrees from the University of Toronto in 2006. He subsequently obtained his Fellowship from the Royal College of Physicians and Surgeons in Infectious Diseases in 2011. After his graduation, Dr. Song joined West Park Healthcare Centre as an Infectious Disease consultant in 2011. His primary area of clinical interest is in Tuberculosis and other mycobacterial infections. He has served as the physician lead of the Antimicrobial Stewardship Program at West Park Healthcare Centre since 2012. Dr. Song currently holds the position of adjunct professor in the Temerty Faculty of Medicine at the University of Toronto.

Anne has built a diverse career spanning hospital pharmacy, pharmaceutical/biotechnology industry, and antimicrobial stewardship. She earned her Bachelor of Pharmacy from the University of Toronto and an MBA from the Schulich School of Business. Her passion for antimicrobials began as a General Medicine Pharmacist at TGH before moving into the role of Coordinator, Unit Dose, and spending over 15 years in Business Development and Licensing roles at GSK, Ontario Genomics Institute, and Arius Research. In 2011, she returned to hospital pharmacy as a Clinical Pharmacist to develop and implement an Antimicrobial Stewardship Program at WPHC. Currently in dual roles as Pharmacotherapy Specialist, UHN ASP and Pharmacist (WPHC), Anne is looking forward to helping expand ASP practices across UHN's Rehabilitation and Complex Continuing Care sites.

Lopa Naik has been a dedicated member of the UHN ASP team since 2010. She received her Master of Computer Application and Bachelor of Science in Mathematics from VNSGU. Lopa's development journey has been a blend of formal education, hands-on experience, and personal reflection. She has completed various certifications during this journey: VB.net certification (Ryerson University), WebSphere Application Server 8.5 Administration on Windows Certification (Web Age Solution), Power BI course (Web Age Solution), and Designing & Implementing Enterprise-Scale Analytics Using Azure (Web Age Solution), and she regularly attends workshops and webinars to stay current in her field. Over the past 10+ years, Lopa has demonstrated a strong track record of self-directed learning to taking on new challenges, perseverance and a robust sense of purpose in her work.

Jenna holds a Bachelor of Medical Sciences from Western University with a specialization in Physiology and Pharmacology, and a Doctor of Pharmacy from the University of Toronto. She completed a hospital pharmacy residency at University Health Network, where she has since worked in General Internal Medicine and Nephrology, prior to transitioning to her current role in Antimicrobial Stewardship. Jenna obtained her Master of Science in Pharmaceutical Sciences, with research focused on antimicrobial stewardship in health professions education. She serves as a guest lecturer at the Leslie Dan Faculty of Pharmacy and is passionate about teaching and precepting learners and trainees in clinical practice. At present, her clinical practice focuses on antimicrobial stewardship in critical care, malignant hematology and general internal medicine.

Shreeya earned her Bachelor of Science from Queen's University and Doctor of Pharmacy from the University of Toronto. She completed her hospital pharmacy residency at Sunnybrook Health Sciences Centre, with rotations in antimicrobial stewardship and infectious diseases. Shreeya then worked as a clinical pharmacist in General Internal Medicine and General Surgery at Sunnybrook before transitioning to her current role with UHN's Antimicrobial Stewardship Program. Her clinical practice focuses on antimicrobial stewardship in allogeneic stem cell transplant, critical care, and internal medicine.

Outside of her clinical work, Shreeya is passionate about education, serving as a clinical instructor and guest lecturer at the Leslie Dan Faculty of Pharmacy, in addition to precepting learners. She also contributes to the Communications Committee of the Canadian Society of Hospital Pharmacists.
