How do I become a living donor?

Step 1: Complete a Living Donor Health History Form. Download or fill our online

Step 2: Submit the completed form in person, by email, mail, website, or fax. Contact information is on the downloadable form. Please include confirmation of your blood type, whether a copy of your blood donor card or lab results, alongside your donor health history form.

Step 3: Our Living Donor Assessment Office will call you once we have reviewed your health history form.

Please review the Living Donor Keratolimbal Allograft Manual prior to the call and consider registering for one of our educational sessions (in the events tab).
