​​Urgent Referral

If your patient needs immediate, urgent care:

Download and complete the Red Blood Cell Clinic Referral Form (PDF).

Indicate in the space provided that the referral is urgent.
3
Fax the completed referral form to 416 340 4559.

Referral Forms

​Standard Referral

To complete the standard referral process:

Download and complete the Red Blood Cell Clinic Referral Form (PDF).

Include the following documents:
  • clinic notes
  • recent bloodwork (MUST include CBC and hemoglobin electrophoresis results)
  • diagnostic testing results (e.g., ferriscan, cardiac MRI)
  • surgical procedure notes (if applicable)

Fax the completed form to 416 340 4559. The form will be reviewed to ensure that it is complete; if incomplete, the form will be returned to you.

The administrative team will call the patient directly to make an appointment. Please ensure that all patient contact information is recent and accurate.

Note: It is extremely important that your patient brings a list of medications that they are taking. These medications will be required at their first appointment.

To ensure your referral is complete, make sure to include:

  • A referral form, filled out in its entirety
  • Clinic notes
  • Recent bloodwork (MUST include CBC and hemoglobin electrophoresis results)
  • Diagnostic testing results (e.g., ferriscan, cardiac MRI)
  • Surgical procedure notes (if applicable)

If this information is not included with the referral form, your referral may be returned as incomplete.
