Urgent Referrals

If your patient needs immediate, urgent care:

Ask to page:

  • The "on-call general surgery resident" for urgent Surgical Oncology referrals
  • The "on-call medical physician" for urgent Medical Oncology referrals
  • The "on-call radiation physician" for urgent Radiation Oncology referrals

If you do not know which physician to refer your patient to, please indicate "next available" and you will be directed to the correct physician.

You must make direct contact with the on-call resident or physician to complete an urgent referral.


Referral Forms

Standard Referrals

To complete the standard referral process:

Download and complete the appropriate referral form. If the referral is for a specific physician, please indicate this clearly on the cover letter.

Include the following documents:

  • Clinical notes
  • Tests, x-rays and any other relevant results, including completed breast biopsy reports and any prior oncology treatments

Fax the complete referral package to the fax number provided on the referral form. Some physicians' offices will send you a confirmation of receipt.

  • For referral to a breast medical and/or radiation oncologist: Fax referrals to 416 946 2900.
  • For referral to a breast surgical oncologist: Fax referrals to 416 946 4429.

The form will be reviewed to ensure that it is complete; if incomplete, it will be returned to you. On this page: Checklist for complete referral

The surgeon or physician's office will call you with information about the patient's first appointment.

Your office is responsible for providing the patient with appointment information, such as time and location, and important documents, test results and imaging that they must bring.


It is extremely important that your patient brings the following reports to their first appointment:

  • Clinical notes
  • Tests, x-rays and any other relevant results, including completed breast biopsy reports and any prior oncology treatments
  • For Surgical Oncology referrals: all breast imaging from the last 5 years on a CD, plus the accompanying radiology reports

Checklist for a Complete Referral

To ensure your referral is complete, make sure to include:

  • A referral form, filled out in its entirety
  • Clinical notes
  • Tests, x-rays and any other relevant results, including completed breast biopsy reports and any prior oncology treatments

If this information is not included with the referral form, your referral may be returned as incomplete.
