​The Bhalwani Familial Cancer Clinic provides genetic counselling and risk assessment services to people with a personal or family history of cancer. We can talk with you about whether the cancer in your family may be hereditary and discuss how you can manage your risk of developing cancer. How to Get Referred

You need a referral from your doctor to become a patient at the Bhalwani Familial Cancer Clinic. Please note that not all referrals will be accepted. If your referral is not accepted, your doctor will receive notice from our clinic.

When making a referral, your doctor will include:

Once the Bhalwani Familial Cancer Clinic has received a referral from your doctor, you will be mailed a letter with a date and time for your appointment. You should receive this letter within 4-6 weeks of being referred. Within the letter you will find a family history questionnaire and a personal medical history questionnaire.

These forms must be completed and returned to our clinic at least 3 weeks prior to your appointment or your appointment may be rescheduled. In some situations, these questionnaires may be provided to you directly by your referring doctor.

          About Your Appointment

          You will be mailed a date and time for your first appointment at the Bhalwani Familial Cancer Clinic. There are three different types of appointments you can have: telephone appointments, virtual appointments, and in-person appointments. Read on to learn what to do before any type of appointment at the Familial Cancer Clinic, and to learn information about your specific appointment type.

          What to Do Before Your Appointment (For All Patients)

          Before any appointment at the Bhalwani Familial Cancer Clinic, you will receive a personal history questionnaire and a family history questionnaire. Please complete both questionnaires and return them to us at least 3 weeks prior to your appointment. If you do not complete your questionnaires, your appointment may be changed to a later date.
