Please note that only physicians may send a referral. Please include the following with your referral:
- A letter of referral
- Clinical notes
- Test reports (blood work, coronary angiogram reports, echocardiogram reports, x-rays and any other relevant results)
If this information is not included, your referral may be returned as incomplete.
It is extremely important to ensure that the patient brings the following as these will be required at their first appointment:
- Coronary Angiograms – on CD
- Echocardiograms – on CD
- Chest X-Rays – on CD
