What We Do

Our outpatient program provides mental health services to adults living in downtown Toronto. We work closely with patients when they are experiencing a crisis.

Our program is based on the Assertive Community Treatment model used throughout North America. It is an intensive, patient-centred program that offers treatment, rehabilitation and support services from a multi-disciplinary team. It has been proven effective in enhancing the quality of life and decreasing the need for hospitalization among people with serious mental illness.

We offer community outreach and support to individuals who may have experienced difficulty maintaining contact with health care workers in the past. Our aim is to help people function better and live more successfully in the community. We do most of our work with patients in their own environments. Our patients help us to develop a treatment plan that is tailored to their needs. We monitor symptoms of illness and promote appropriate medical treatment.

We also can help with housing, finances, drug and alcohol abuse, medication, family issues, legal problems, rehabilitation and vocational goals. Our team can provide education and skills training needed to live successfully in the community. Additional education and support is offered to family members of our patients.
