About Scope

SCOPE is a virtual interprofessional health team that supports primary care providers through a single point of access. Family physicians and nurse practitioners registered with SCOPE can connect to local specialists, imaging, and community services, to serve their patients with complex care needs.​

For immediate consultation or advice from a hospital internist who can also refer or assess your patient in the Acute Ambulatory Care Clinic at WCH​.

For access to a CCAC care coordinator and at-home supports.​

F​or general advice and assistance navigating all and miscellaneous healthcare resources for your complex patients.

Additional SCOPE services can be accessed through option 3, including:

  • RN Health Coaches—for one-on-one individualized nursing support for your patients to improve chronic disease self-management
  • Telemedicine IMPACT Plus—for rapid access to interprofessional consultation with a psychiatrist and geriatrician or internist, along with coordinated care planning and follow-up supports from a nurse
  • Substance Use Network—for support, advice, or care from addictions medicine physicians at WCH for your patients requiring substance use management
  • Transitional pain service—for specialized pain management of your pre- and post-surgical patients

F​or same-day consultation from a radiologist who can also escalate your urgent imaging request.
