Hi everyone,
Today, we celebrated the official opening of the Toronto Western Hospital's Family Health Team (FHT) with the Honourable David Caplan, Minister of Health and Long-Term Care. We are thrilled to be one of only nine FHTs selected in the Toronto Central LHIN, which clearly speaks to UHN's expertise in providing patient-centred care. Unlike a traditional family physician's office, a FHT brings together multiple health providers (such as a family physician, a dietician, a nurse practitioner and other allied health professionals) under one roof. For patients, this means comprehensive primary care around the clock.
Under the leadership of Dr. Jeff Bloom, Family Physician-in-Chief and Kathy Davison, Acting Executive Director, the TWH Family Health Team will focus on providing complex disease management. Patients living in the city's downtown community will have access to this dynamic range of primary care services. For patients who are currently with the Toronto Western Family Health Centre, the transition to the new model will be seamless.
