Request for Documentation in Accessible Format

UHN is committed to providing accessible information and communication to all, and recognizes that persons with disabilities may require information in accessible formats.

All documents required by the Customer Service Standard and the Integrated Accessibility Standards Regulations (IASR) are available upon request, subject to the Freedom of Information and Protection of Privacy Act (FIPPA). When providing these documents, UHN will do so in the format requested.

When emailing your request, please include the following information:

  • Your full name
  • Email address
  • Mailing address (optional)
  • Telephone number
  • Fax number (optional)
  • Name of required document
  • Format Requested (including but not limited to: large print, hard copy, email, digital, Braille translate. For digital or video request, please indicate closed captioning or descriptive video)
  • Date information is required (note that for closed captioning and/or descriptive video, we require at least two weeks notification per request)

In some cases, we may need to consult with the person making the request to discuss other possible formats in order to provide its services within its capabilities. Alternative formats will be provided within a reasonable time frame, or UHN will provide an explanation for the delay if longer than 15 business days.
