Referrals

Outpatients will require a referral from their family physician for assessment by West Park's Lower Limb Preservation Clinic. Referral by fax only.

What We Do

A comprehensive, interdisciplinary consultation designed to prevent limb loss. The clinic focuses on identification of limb-threatening risk factors, optimization of diabetes management and cardiovascular risk, off-loading strategies, patient education and coordination of referrals for necessary diagnostics and specialty care.

Program Hours, Attendance and Transportation

Wednesday afternoons
West Park does not provide transportation services. We can provide information about community transportation services, if needed.

Eligibility Criteria

18+ years old
Poor foot health (skin/nail changes, loss of sensation)

Exclusion Criteria

Palliative/Life expectancy less than 3 months
