 What We Do

We are a specialized service providing mental health support to patients experiencing health concerns that complicate a concurrent physical health illness.

We work with patients who require:

  • Follow-up after receiving mental health care from UHN's inpatient medical psychiatry consult service during an inpatient medical or surgical admission; or
  • Mental health care to optimize their physical health as they receive outpatient care from UHN's medical or surgical services.

We are an interdisciplinary team of psychiatrists, nurse practitioners and social workers that provide up to 6 months of treatment to eligible patients referred to the clinic by a UHN physician or nurse practitioner. Treatment options may include:

  • Psychiatric assessment
  • Pharmacotherapy
  • Psychotherapy
  • System navigation
  • Connecting to social assistance programs, such as income support, housing and food security
  • Case management in collaboration with other providers

Patients referred to the clinic are matched to the appropriate level of care depending upon their level of need. Lower-need patients get the least intensive interventions, like help navigating the health care system or self-directed resources. Those with high needs receive more intensive care, which could include talk therapy and pharmacological management. Care is regularly reviewed and stepped up or stepped down based on patient need and level of distress.
