Volunteer Reference Form
Attend an Interview
Once your application is reviewed, you will be invited to an interview. A Volunteer Health Assessment form will either be given to you during the interview or emailed to you after the interview if you are conditionally accepted as a volunteer. At Toronto General Hospital (TG), Toronto Western Hospital (TW) and Princess Margaret Cancer Centre (PM), a conditional acceptance will also be emailed to you after the interview. It will contain instructions for the next steps of the onboarding process. At Toronto Rehab, you will be told at the end of the interview if you have been accepted and asked to fill out a paper form of the Criminal Record Check (CRC) and submit a copy of your ID.
For the protection of our patients, all volunteers 18 years of age and older are required to complete a Criminal Record Check (CRC). If you are interested in becoming a volunteer at Princess Margaret Cancer Centre (PM), Toronto General Hospital (TG), Toronto Rehab (TR) or Toronto Western Hospital (TW), you must agree to a Criminal Record Check (CRC). There will be no charge to you for the CRC. During the interview, the process for the Criminal Record Check will be explained to you.
Complete a Health Clearance
If you are accepted to our volunteer program you will be expected to complete a health assessment with the help of your family physician (or walk-in clinic if you do not have a doctor), showing proof of immunity to measles, mumps, rubella and varicella (chickenpox ). You will also need to undergo a 2 step TB skin test. You cannot volunteer if this information is not provided. This is a requirement under the Ontario Hospitals Act. It is for the protection of our patients as well as for you.
Please email your completed Volunteer Health Assessment form (which includes your Volunteer Immunization Status Record) to the Occupational Health and Safety Department email indicated in the conditional offer from TG, TW, PM or instructions given to you by TR Volunteer Resources staff.
The Occupational Health and Safety Department will review your Volunteer Health Assessment form. Once it has been checked and all is in order, you will cleared by the Occupational Health and Safety Department and they will email you a notification indicating that you have been cleared. Volunteer Resources will receive a copy of the email.
Please note: The Volunteer Health Assessment Form (which includes the Volunteer Immunization Status Record) must be completed by your family doctor/walk-in clinic doctor. UHN does not reimburse any fees your family doctor or walk-in clinic doctor may charge for this service.
Complete Criminal Record Check
At Toronto Rehab (TR), Volunteer Resources staff will ask you to fill out a paper form of the Criminal Record Check (CRC) and submit a copy of your government issued ID when you first come in for your interview (Step 4). Once Volunteer Resources staff receive the email notification from the Occupational Health and Safety Department indicating you are cleared, they will process the Criminal Record Check (CRC). At Toronto General Hospital (TG), Princess Margaret Cancer Centre (PM) and Toronto Western Hospital (TW), Volunteer Resources staff will email you a link for your Criminal Record Check (CRC) once they have received the email notification from the Occupational Health and Safety Department indicating that you have been cleared. After receipt of a clear Criminal Record Check (CRC), Volunteer Resources staff will email you instructions and a link for Part 1 of the New Volunteer Orientation.
