Referrals

Toronto Rehab's outpatient musculoskeletal program is currently accepting individuals with the following conditions:

  • fractures
  • bone and/or tendon surgical repairs
  • musculoskeletal spinal surgery
  • joint replacement revision surgery
  • musculoskeletal injuries and/or surgery resulting in an inpatient rehab admission

In order to be eligible for services in the outpatient program the:

  • surgical or injury date should be within the last three months
  • patients should have transportation to and from their appointments
  • patients should be independent with self-care (or have an attendant accompany them to their appointment for assistance)

Please note: the program does not see individuals whose injuries were sustained in a motor vehicle or workplace accident in which there is motor vehicle or WSIB insurance coverage to fund therapy services.

What We Do

Toronto Rehab's outpatient musculoskeletal program specializes in the rehabilitation of individuals with complex musculoskeletal conditions to help optimize their function and independence. Our interdisciplinary team treats individuals with the following conditions:

  • Fractures, including hip fractures
  • Trauma
  • Bone and/or tendon surgical repairs
  • Musculoskeletal spinal surgery
  • Joint replacement revision surgery
  • Musculoskeletal injuries and/or surgery resulting in an inpatient rehab admission
