Referrals

To make a referral, please contact the Service Coordinator at 416 597 3422 ext. 3486, or download the referral form and fax it to 416 597 7164. In addition to the completed referral form, make sure to include the following:

  • Clinical notes including ambulance and ER reports
  • Test results, x-rays and other relevant results

Our office will call the patient with information about the patient's first appointment.


It is extremely important to ensure that your patient brings the following information as these will be required at their first appointment.

  • A list of all their medications from th​eir pharmacy
  • Disks from any scans (e.g., CT, MRI) completed since their injury

What We Do

The Complex Injury Outpatient Rehab Service offers rehabilitation for people with complex acquired brain injuries, musculoskeletal injuries and stroke.

We focus on helping individuals return to active lives as soon as possible. Services are provided in both an outpatient setting and in the community, and we work closely with patients, families, caregivers, insurers and community agencies.

Types of Conditions Treated

We assess and treat people with multiple fractures, moderate to severe acquired brain injuries or multiple system injuries. These individuals are ready for outpatient rehab after being in acute care or an in-patient rehabilitation centre. We also welcome people who are already living in the community who still have ongoing goals to be more independent and active, whether it’s returning to work, going back to school or participating in the community.

Cost of Services

Services provided through Complex Injury Outpatient Rehab are usually not covered by OHIP. In most cases, services are covered by third parties such as motor vehicle insurance, the Work and Safety Insurance Board (WSIB) and/or extended health benefits.
