Referrals

Referrals are accepted from physicians or nurse practitioners. We also offer phone consultations to the public or to clinicians in the community. The referral form can be faxed to 416 597 7019

Our office will contact you with information about the patient's first appointment.

For patients with OHIP coverage: there is no fee for consultation, assessment, prescription or initial training. But there are costs associated with equipment. Your patient may be eligible for financial assistance for communication equipment through Ontario's Assistive Devices Program (ADP).

If your patient meets the ADP's criteria, they can participate in an equipment leasing program or receive funding to purchase a device (up to 75% of the cost). The remaining costs are their responsibility. If they are not eligible for ADP funding, they must pay for any equipment they require.

Please note that clinic appointments may be virtual visits. The clinic team will let you know what type of appointment is appropriate for your care. Learn more about virtual visits at UHN »

What We Do

For people with neurological conditions such as acquired brain injury (ABI), stroke, ALS or Parkinson's disease, speaking or writing can sometimes be difficult. Our clinic offers solutions to these communication challenges.

Each individual is different, so we work to find solutions that work for each specific person. These may include alternative ways of using computers, such as relying on adaptive software and hardware. People might use electronic devices that have artificial voice output or use picture/word books.

We complete a comprehensive assessment of each person’s needs and then look for innovative solutions. Sometimes it’s a matter of finding the right equipment. It may mean training and support, or help in getting funds from the Assistive Devices Program of the Ministry of Health and Long-Term Care.
