Referrals

You do not need a referral to obtain an appointment at our clinic. Please call us at 416 349 4800 ext. 7067 to book your appointment. We can usually schedule you to come in within a couple of weeks. Please note that most of our services have accompanying fees as most are not covered by OHIP. You can visit our clinic on a walk-in basis to:

  • Have your hearing aid cleaned or sent out for repair if needed.
  • Purchase batteries or assistive listening devices.

If a physician would like to refer patients for one of our services, please check off the appropriate boxes on the Munk Hearing Centre Referral Form and fax it to 416 340 5149.​​

What We Do

The Munk Hearing Centre is the full-service hearing aid dispensary at UHN. All of our audiologists are registered with the College of Audiologists and Speech-Language Pathologists of Ontario (CASLPO) and can reco​mmend the hearing health care solution that is best for you based on your specific needs. We will provide you with a complete hearing aid evaluation, fitting and follow-up, as well as cleaning and care for your hearing aid.

We work closely with the Centre for Advanced Hearing and Balance Testing and the Otolaryngology, Head and Neck Surgery Clinic to ensure that your needs are met.​

Tests and Services

  • Hearing aid evaluations and demonstrations
  • Counselling about hearing loss and prevention
  • Hearing aid prescriptions, dispensing and fitting and follow-up
  • Coordination of benefits coverage through government programs, including Veterans Affairs Canada, the Workplace Safety and Insurance Board (WSIB) and the Assistive Devices Program (ADP)​
  • Assistive listening devices (ALDs), such as FM systems, amplified telephones and alerting devices
  • Custom earplugs
  • Hearing aid batteries
  • On-site hearing aid repairs and servicing​​
  • Tinnitus management​
