Referrals

Please note that only physicians may send a referral. All referrals can be sent to the Toronto Academic Pain Management Institute:

  • TAPMI referral form
  • All relevant clinical notes
  • Tests and any other relevant results including imaging films or CDs (x-ray, CT, MRI, ultrasound)
  • List of interventions performed on the patient by other providers to relieve pain
  • List of medications previously or currently prescribed for the management of the patient's pain condition
    • Please note the clinic will not take over prescribing opioids

Our office will contact the patient directly with information about the patient's first appointment.

Comprehensive Integrated Pain Program (CIPP): What We Do

We treat patients who experience chronic, neuropathic and musculoskeletal pain and cancer pain conditions. People who suffer neuropathic pain – also known as nerve pain – have nerve fibres that have been damaged or are not functioning as usual. Neuropathic pain can be debilitating to those who live with these conditions.

We provide the following treatments for patients with neuropathic and musculoskeletal pain:

  • Diagnostic and therapeutic nerve blocks, including epidural steroid injections, spine facet joint blocks, injections into the hip, knee, shoulder or other joints
  • Radiofrequency ablations for spinal and limb joints
  • Regenerative medicine therapies (platelet-rich plasma) for musculoskeletal pathologies
  • Neuromodulation Therapy including spinal cord stimulation
  • Transitional Pain Services
  • Spontaneous Intracranial Hypotension
  • Intravenous infusions of lidocaine and/or ketamine
  • Interventions for cancer pain including neurolytic therapy and intrathecal drug delivery
