Referrals

Now Accepting Ocean eReferrals

This clinic is now accepting referrals through Ocean eReferral to help everyone save time and increase efficiency.

Our office will acknowledge the referral by fax or phone as soon as possible. We will provide the approximate wait time (as able) and indicate if any additional information is required to complete the referral. If an appointment date and time is available it will be communicated to the patient directly. If the referral is declined, a reason will be provided to the referring physician.

Please note that clinic appointments may be virtual visits. The clinic team will let you know what type of appointment is appropriate for your care. Learn more about virtual visits at UHN »

What We Do

The Foot and Ankle Rapid Access Clinic (RAC) Clinic is a new model of care created to help assess and manage Ontarians with foot and ankle pain and related symptoms. The clinic provides patients with rapid access, thorough assessment and personalized self-management plans to empower patients to take control of their foot and ankle pain.

The following conditions are seen in our Foot and Ankle RAC clinic:

Ankle

  • Instability/sprain
  • Osteorchondral lesion
  • Ankle/subtalar arthritis
  • Deformity/tendon contracture
  • Second Opinion – with detailed reason for referral

Foot

  • Severe hallux valgus/bunion
  • Hallux rigidus/1st MTPJ arthritis
  • Midfoot arthritis
  • Pes planus/flatfeet
  • Accessory navicular/tarsal coalition
  • Pes cavus/high arch feet
  • Second Opinion – with detailed reason for referral

Acute foot and ankle injuries, including fractures, should be referred directly to TWH Fracture Clinic (Fax: 416 603 6752).

At the RAC clinic, patients are assessed by an Advanced Practitioner who has advanced training in arthritis and musculoskeletal care through the Advanced Clinician Practitioner in Arthritis Care (ACPAC) Program in addition to training directly with the specialist orthopedic surgeon. Patients who require surgical intervention will be referred to an orthopedic surgeon. Patients who do not require surgery will be provided with an evidence-based self-management plan, streamlined access to specialists and diagnostic services when indicated, and directed to local services if needed.

Services

  • Foot and ankle assessment – may include diagnostics (x-rays, MRI, CT scan)
  • Self-care management – e.g., education, exercise prescription, bracing, orthotics
  • Orthopaedic surgery – e.g., arthroscopic, ligament repair, hindfoot/forefoot reconstruction
