If you are a registered SCOPE provider, you can access SCOPE resources including referral forms, news, and continuing medical education events. Select a category to begin.
SCOPE is a virtual interprofessional health team that supports primary care providers through a single point of access. Family physicians and nurse practitioners registered with SCOPE can connect to local specialists, imaging, and community services, to serve their patients with complex care needs.
For immediate consultation or advice from a hospital internist who can also refer or assess your patient in the Acute Ambulatory Care Clinic at WCH.
For access to a Home and Community Care Coordinator care coordinator and at-home supports.

SCOPE is currently expanding in the Mid-West, West, North and East Toronto. For a limited time you can sign-up to apply for access to SCOPE services. You can currently sign-up for the Mid-East Toronto (St. Michael’s Hospital) wait-list. SCOPE is evaluated as part of a Quality Improvement initiative.
