How To Get Referred

Referral Information for Patients

A respite visit must be booked in advanced through our admissions office. For more information, including availability and rates, please call 416 597 3422 ext. 2504.

The patient must be age 18 or older and meet the criteria for complex continuing care.

How to Get Referred

You need a referral from your health care team become a patient at the Respite Service.

When making a referral, your doctor will include:

  • A referral form
  • Clinical notes
  • Tests, diagnostic reports and any other relevant results

How Long Does a Referral Take?​

After we receive your referral, and if it meets our criteria, our Admissions Service Coordinator will contact your health care team to confirm your admission date and provide information about your inpatient stay.

Admission Criteria

The patient must be age 18 or older and meet the criteria for complex continuing care.

The Respite service is appropriate for patients who:

  •  Are currently in the community
  • Plan to return to the community
  • Require a short (maximum 2-week) stay
  • Meet general CCC criteria

Referral

To submit a referral, first download and complete the Inpatient Rehab/CCC Referral Form​.

Once you have completed the form, fax it to 416 530 2470.​

Once we receive your referral, we will review the referral to ensure that it is complete; if incomplete, processing may be delayed.

We will contact you to confirm the admission date and provide information about the patient's stay. Your office is responsible for providing the patient with the information about their admission and stay.
