How To Get Referred

Referral Information for Patients

You need a referral from your health care team to become a patient of the Low Tolerance Long Duration Rehabilitation Inpatient Unit.

When making a referral, your health care team will include:

  • A referral form
  • Clinical notes
  • Tests, diagnostic reports and any other relevant results

How Long Does a Referral Take?

After we receive your referral, our Admission Service Coordinator will contact your health care team within 48 hours with a response.  If your application is accepted, your health care team will be contacted as soon as an appropriate bed is available to confirm your admission date and provide information about your inpatient stay.

​Referral Information for Health Care Providers

​To submit a referral, you may use the Resource Matching and Referral process (RM&R), or download and complete the Inpatient Rehab/CCC Referral Form​.

Once you have completed the form, fax it to 416 530 2470.

After we receive your referral, we will review it to ensure that it is complete and contact you within 48 hours with a response. If incomplete, the processing of your form may be delayed.

If the application is accepted, and once an appropriate bed is available, we will contact you to confirm the admission date and provide information about the patient's stay. Your team is responsible for providing the patient with the information about their admission and stay.
