How to Get Referred

Referral Information for Patients

You need a referral from your health care team to become a patient of the Complex Medical Service.

When making a referral, your health care team will include:

  • A referral form
  • Clinical notes
  • Tests, diagnostic reports and any other relevant results

How Long Does a Referral Take?

After we receive your referral, our Admissions Service Coordinator will contact your health care team to confirm your admission date and provide information about your inpatient stay.

​Referral Information for Health Care Providers

​​​​​​​​To submit a referral, you may use the Resource Matching and Referral process (RM&R), or download and complete the Inpatient Rehab/CCC Referral Form​.

Once you have completed the form, fax it to 416 530 2470.

After we receive your referral, our Admissions Service Coordinator will contact you to confirm an admission date and provide information about your patient's inpatient stay.

 

Checklist for a Complete Referral

​In addition to the completed referral form, make sure to include:

  • Clinical notes
  • Tests, diagnostic reports and any other relevant results
