 What We Do

The Geriatric Medicine Clinic looks after people who have complex conditions associated with aging. These could be memory loss, mobility problems and falls, bowel or bladder difficulties, poor nutrition or unexplained weight loss, as well as other challenges coping with multiple illnesses and medications.

We provide special assessments for older adults, and when it's needed and appropriate, we put our patients in touch with community support services.

Toronto Rehab does not provide transportation services to and from the hospital. If you are unable to access the hospital on your own, we can connect you with community transportation services such as Wheel-Trans.
