 What We Do

The Geriatric Day Hospital Service is an outpatient rehabilitation program that helps older adults with multiple health problems to be more active in the community.

We conduct detailed health assessments, develop personalized treatment plans for you, offer health education and put you in touch with community services and programs. We also provide comprehensive discharge planning and support.

As one of our patients, you'll have therapy structured around clear rehab goals. Therapy may include both 1:1 and group-based sessions. Our program lasts 7 – 10 weeks.

Geriatric Day Hospital Transportation

Toronto Rehab does not provide transportation services to and from the hospital. If you are unable to access the hospital on your own, we can connect you with community transportation services such as Wheel-Trans. You can find out more about getting to and from UHN here.
