You need a referral from your doctor to become a patient at the Bariatric Clinic.
You are eligible to be assessed for bariatric surgery if you:
When making a referral, your doctor will submit a referral form to the
Bariatric Registry, a central system used to refer patients to the bariatric centre closest to them. Each centre in the registry is
formally designated as a bariatric centre by the Ontario Ministry of Health and Long-Term Care.
You will receive a letter that tells you when and where to come for the orientation session. Your orientation session will take place within 3 months of us receiving your referral.
We will provide you with information about your first assessment appointment(s) at the end of your orientation session.
