How to Get Referred

​​​​You need a referral from your doctor to become a patient at the Bariatric Clinic.

You are eligible to be assessed for bariatric surgery if you:

  • Have a Body Mass Index (BMI) above 40 or above 35 with obesity-related medical problems (including diabetes, high blood pressure, sleep apnea or joint problems)
  • Have made previous weight-loss attempts
  • Have been a non-smoker for at least 6 months
  • Are not misusing substances
  • Are motivated and willing to make eating and lifestyle changes
  • Are psychologically and medically fit for surgery
  • Are willing to commit to follow-up for 5 years after your surgery

When making a referral, your doctor will submit a referral form to the Bariatric Registry, a central system used to refer patients to the bariatric centre closest to them. Each centre in the registry is formally designated as a bariatric centre by the Ontario Ministry of Health and Long-Term Care​.


How Long Does a Referral Take?

You will receive a letter that tells you when and where to come for the orientation session. Your orientation session will take place within 3 months of us receiving your referral.

We will provide you with information about your first assessment appointment(s) at the end of your orientation session.
