Urgent Referral

If your patient needs immediate, urgent care, please call 911 or direct your patient to proceed to the nearest Emergency Room.

To complete an urgent referral, you must make direct contact with the appropriate physician. If you do not know which physician to refer your patient to, please call Kim Moffat (Administrative Assistant to the Chief of Urology) at 416 946 2899 or call UHN Locating at 416 340 3155 and ask for the urologist on call. You will be directed to the correct physician.

Please note: All suspected cases of testicular torsion, priapism, sepsis secondary to urinary obstruction, trauma, testicular masses and renal mass with inferior vena cava (IVC) involvement are considered urgent referrals.

Standard Referral

To complete the standard referral process, fax a letter of referral and related documents (see "Checklist for a Complete Referral" below) to us at 416 946 6590.

Our office will call you with information about the patient's first appointment.

Your office is responsible for providing the patient with the appointment information, including time and location, and important documents, test results and imaging that they must bring.

It is extremely important to ensure that your patient brings the following as these will be required at their first appointment.

  • All relevant CT/MRI/bone scans on CD
  • All relevant correspondence

Checklist for a Complete Referral

Please write the full name of our clinic on your referral letter to ensure your referral is triaged appropriately.

In addition to a letter of referral, make sure to include:

  • CT/MRI/bone scan reports
  • Relevant clinic notes
  • Relevant lab test results

If this information is not included with the referral letter, your referral may be returned as incomplete.
