If your patient needs immediate, urgent care, please indicate that on the referral form and provide a summary of the patient's case.

Please note: All cases of fever in the returned traveller are considered a medical emergency, and your patient should first present to their local emergency room for exclusion of malaria and other potentially life-threatening travel-acquired illnesses.

Standard Referral

To complete the standard referral process, fax your referral form to the Tropical Disease Unit at 416 340 3260.

Referrals are triaged by urgency of the complaint.

Our office will call you with information about the patient's first appointment.

Your office is responsible for providing the patient with the appointment information, including time and location, and important documents, test results and imaging that they must bring.

It is extremely important to ensure that your patient brings the following, as these will be required at their first appointment.

  • CT scan, MRI, x-ray, or ultrasound reports related to their current complaint

Checklist for a Complete Referral

In addition to the completed referral form, make sure to include:

  • Results of investigations related to the current complaint
