​​Standard Referral

Each physician in our division runs an independent medical practice according to his/her clinical interests and expertise. Contact the individual doctor who you would like your patient to see. You'll be instructed on how to complete their standard referral process.

Referrals addressed to the "Division of Endocrinology" will not be accepted.

Your office is responsible for providing the patient with the appointment information, including time and location, and important documents, test results and imaging that they must bring.


Urgent Referral

If your patient needs urgent care, you should contact the physician directly, but most urgent medical issues cannot be treated in an ambulatory setting. In these situations, please have your patient proceed to the emergency department at UHN.

Please instruct your patient to proceed to the emergency department at UHN for the following urgent referrals:

  • Diabetic ketoacidosis
  • Diabetes with severe hyperglycemia, dehydration and metabolic decompensation
  • Hyperthyroidism with severe symptoms of thyrotoxicosis
  • Pituitary apoplexy
  • Hypertensive emergency from hyperaldosteronism or pheochromocytoma
  • Severe adrenal insufficiency (adrenal crisis)


Checklist for a Complete Referral

In addition to a letter of referral, make sure to include:

  • Relevant clinical history
  • Prior consultation letters
  • Relevant test results
  • A current medication list
  • Patient demographics such as contact information, OHIP number etc.


If this information is not included with the referral form, your referral may be returned as incomplete.
