Please note: All Status Epilepticus referrals are considered urgent. This will be assessed accordingly by the neurologist accepting the patient.​

Standard Refer​ral

To complete the standard referral process, please fax a referral letter to the Epilepsy Clinic at 416 603 5768 and address it to one of the following physicians:


The information will be reviewed to ensure that it is adequate, otherwise your office will be notified of the omissions and the appointment will be booked when complete information is received.

Our office will call you with details about the patient's first appointment.

​Your office is responsible for providing the patient with the appointment information, including time and location, and important documents, test results and imaging that they must bring. ​

It is extremely important to ensure that your patient brings the following, as these will be required at their first appointment.


  • EEG Reports
  • Epilepsy Monitoring Unit Report (If done)
  • MRI Scans (on disc)​

Checklist for a Complete R​​eferral​​​

In addition to the completed referral form, make sure to include:​

  • Clinical notes from previous consultants
  • Ima​ging Studies (CT, MRI)
  • Reports of any abnormal laboratory tests

If this information is not included with the referral form, your referral may be returned as incomplete.
