​​​​​Urgent Referral ​​​​​​

If your patient needs immediate, urgent care, please refer them to the Emergency Department.

If you have a patient that you think requires semi-urgent assessment, but does not need the Emergency Department, a referral should be faxed 416 603 4348. The referral should contain sufficient information to permit assessment of the urgency of the case, including history and physical/laboratory findings of concern. 

You must make direct contact with the appropriate physician to complete an urgent referral.

If you do not know which physician to refer your patient to, please call Eve Pereira in the Administrative Offices at 416 603 5001. You will be directed to the correct physician.


Standard Referral​​

Referrals should be addressed to the attention of the Rheumatology STAT Clinic or Dr. Joan Wither and faxed to 416-603-4348. All referrals are triaged by Dr. Joan Wither based upon urgency and severity. The clerical staff will then contact the patient with the appointment details. 

When referring your patient to Dr. Wither, please provide the following:

  • ​A referral letter
  • Clinical notes
  • Lab and x-ray findings

It is extremely important to ensure that your patient brings releva​nt reports, tests, scans, as well medications he/she is currently taking, as these will be required at their appointment. 

Our office will call you with information about the patient's first appointment. Your office is responsible for providing the patient with the appointment information, including time and location, and important documents, test results and imaging that they must bring. It is very important that your patient brings all relevant documentation as well as any medications they are currently taking.
