Jay Esguerra’s palliative care patient sat immobile. He was in the multi-organ transplant unit at Toronto General Hospital – bed-ridden and unable to fulfill tasks of day-to-day living. The patient was told he had months to live. His last wish was to walk out of the hospital and spend his remaining days with his wife and grandchildren at home. Click on the video player above to hear Jay's story about this patient.
