Stroke Rehab Inpatient Service - Referrals

​​​Admission Criteria

The Stroke Rehab Inpatient Service is appropriate for patients who:

  • Have a confirmed diagnosis of a recent thrombolic, embolic or hemorrhagic stroke
  • Are medically stable
  • Are at least 18 years of age
  • Show the potential to benefit from an intensive rehabilitation program ( i.e. follow commands, demonstrate carry-over of learning )
  • Have clear/identifiable rehab goals that can be achieved within the targeted length of stay
  • Demonstrate the willingness and ability to actively participate in the rehab program
  • Are able to tolerate a minimum of 3 hours of rehab activity daily
  • Do not demonstrate behaviours that would pose a risk to self or others
  • Do not require constant supervision by an attendant

Referral

Our Stroke Rehab Inpatient Service has a single-point entry system. All referrals are screened by our Stroke Rehab Inpatient Service Coordinator, who will discuss the referral with our admissions review group as needed.

Referrals should be made through the eStroke Referral system.

If you are not using this system, download and complete the Inpatient Rehabilitation Referral Form and the Stroke Service Referral Form​.

Once you have completed the forms, fax them to our Stroke Rehab Inpatient Service Coordinator at 416 597 7141.

Once we receive your referral, we will review the referral to ensure that it is complete; if incomplete, the forms will be returned to you.

We will contact the referral source to confirm the admission date and provide information about the patient's stay. Your office is responsible for providing the patient with the information about their admission and stay.​


Checklist for a Complete Referral

In addition to the completed referral forms, make sure to include:

  • Most recent neuroimaging to confirm stroke
  • Any relevant medical consultations pertaining to the stroke
  • Hospital discharge summaries or therapy notes

If this information is not included with the referral forms, your referral may be returned as incomplete.​
