Guide for Physicians Referring to the Palliative Care Clinic (Outpatient)

Note: the Palliative Care Clinic only accepts referrals from UHN physicians for Princess Margaret cancer patients.

Urgent Referral

If your patient needs immediate, urgent advice for symptom management, please enter an order for "Outpatient Referral to Palliative Oncology" through Epic AND page the Palliative Care fellow on-call to discuss. Please note we generally can only provide urgent consultations until 4:00pm.

Standard Referral

To complete the standard referral process:

Enter an order to "Outpatient Referral to Palliative Oncology" through Epic.

Our physician team will triage the referral.

The office will contact you with the patient’s appointment information. You must then contact the patient to tell them about the appointment.
