​​​​Guide for Physicians Referring to the Hepatobiliary (Liver and Pancreas) Surgical Clinic

​​ ​Pancreas Cancer referrals should be sent directly to the McCain Pancreas Clinic.

Send all other liver and pancreas surgical referrals to the HPB referral fax line: 1 866 547 2844. The surgeon on call will review and triage the referrals.

To complete the standard referral process:

Download and complete the appropriate referral form.

Fax the completed form to the appropriate fax number as indicated above. We will contact your office to confirm receipt.

The form will be reviewed to ensure that it is complete; if incomplete, additional information will be requested.

In addition to the completed referral form, whenever possible, please include:

  • Letter (with History & physical; co-existing conditions; allergies; previous malignancy; medications)
  • Pathology reports
  • Operative reports
  • Surgical procedure notes
  • Imaging CT/US/MRI/XRAY
  • Blood work
