If your patient needs an emergent coronary angiogram or coronary angioplasty, please contact the CICU Senior directly at 416 340 4066. Alternatively, please page the Senior on call for the CICU through locating at 416 340 3155.

Urgent Refe​rral

If your patient needs an urgent coronary angiogram or coronary angioplasty within 24 to 48 hours, please call the Cardiac Triage Office at 416 340 5184.

Referral Form

For all urgent and standard referrals, first download and complete theCath_Referral_Form_UHN.pdf.
Once you have completed the form, fax it, along with any relevant tes​ts, to 416 340 3788. On the referral form, please include your fax number so we may provide you with a preliminary report in a timely manner.

Please ensure that the form is complete; if incomplete, we will need to contact you to obtain all relevant clinical and patient safety information to complete the booking in a timely manner. We are also required to submit complete forms to CCN for the provincial database and appreciate your help in collecting and sending all the important information. Your office is responsible for providing the referral documents, test results and imaging that they must bring and for arranging needed blood work. It is extremely important to ensure that your patient has had the following tests prior to the procedure date:

  • Blood tests: including CBC, Electrolytes, Creatinine, Blood Sugar (within 90 days of cath)
  • In certain circumstances we may require additional blood tests or chest x-ray, we will let your office know
  • ECG (within 90 days of cath)

Booking – What Will Happen

For urgent referrals, our office will call you with the procedure date and time.
For elective referrals, our office will call the patient to inform them of the appointment and then notify your office. Alternatively, feel free to call the Triage Office while the patient is in your office and receive a procedure date provided the referral form is complete and blood work is done or arranged. You should feel free to contact the offices of any UHN cardiologists who perform caths directly if that is the route you prefer. The triage office is responsible for providing the patient with the appointment information, including time and location.

In addition to the completed referral form, make sure to include (reports/tests/scans/clinics notes, etc.)

  • Recent consult note or referral note
  • Blood test results (within 9​​0 days of cath)
  • Detailed note of previous bypass surgery
  • Stress test and/or perfusion scan, if available
  • Echo, if available
