Heart Failure and Assessment for Heart Transplant

If your patient is experiencing heart failure and needs assessment for a heart transplant, please refer your patient to the Heart Function Clinic by completing thereferral form for the Ted Rogers Centre of Excellence in Heart Function (formerly called the Advanced Heart Failure Program.) Please fax your completed referral to 416 340 4134.

If your patient is experiencing severe heart failure or cardiac symptoms and needs urgent/emergent care, please make direct contact with the heart failure/heart transplant physician on call by calling UHN Locating at 416 340 3155.​​​

Heart Transplant

If you are a physician who requires information regarding a heart transplant recipient, please contact the Heart Transplant Clinic directly by phone at 416 340 4800 ext. 6997. For more immediate concerns, please contact the heart transplant cardiologist by calling UHN Locating at 416 340 3155.

Please note: All patients with cardiogenic shock are considered emergent referrals.


Standard Referral

To complete the standard referral process, first download and complete thereferral for​m​​ for the Ted Rogers Centre of Excellence in Heart Function​ (formerly called the Advanced Heart Failure Program).

Once you have completed the form, fax it to 416 340 4134.

The form will be reviewed to ensure that it is complete; if incomplete, the form will be returned ​to you.

Our office will contact the patient to provide information about the patient's first appointment.

It is extremely important to ensure that your patient brings the following where available as these will be reviewed at their first appointment:

  • ​2D echocardiogram with Doppler
  • Abdominal ultrasound
  • Angiogram and CD
  • Bone density scan
  • Chest x-ray
  • Coronary bloodwork
  • CT scan of chest
  • ECG
  • Heart stress tests
  • Hemodynamic monitoring
  • Urine tests
