If your patient needs immediate, urgent care, please do the following:
Submit a referral form via fax including the reason for urgency to facilitate triage.
Please note, we are an appointment based service and not an urgent care service. Our staff can speak with you directly to determine if the referral to our service is appropriate.
To complete the standard referral process, first download and complete the
Community Mental Health Clinic referral form.
Once you have completed the form, fax it to 416 603 5490.
The form will be reviewed to ensure that it is complete; if incomplete, the form will be returned to you.
Unfortunately, due to limited resources, referrals with active litigation including but not exclusive to, MVAs and WSIB involvement will not be accepted. As well, those requesting a consultation for the purpose of submitting information for immigration related issues, disability application or forensic assessments will not be accepted. Please contact our office if you are unsure as to whether or not a referral is appropriate.
Our office will usually contact your patient with the first appointment date unless your referral specifies that the first appointment be arranged through your office.
In cases when the appointment is arranged through you, your office is responsible for providing the patient with the appointment information, including time and location, and important documents, test results and imaging that they must bring. You may choose to fax us the information ahead of time to 416 603 5490.
